Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery from 10am to 6pm

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our catchment area

You will need to show us proof that you have changed your date of birth. This would be your updated birth certificate.
